What Is Tonometry for People with Diabetes?
Tonometry is a simple test that measures intraocular pressure (IOP) — the pressure created by the fluid inside your eye.
Your eye is always making a clear fluid called aqueous humor. When this fluid drains the way it should, your eye pressure stays steady. But if the drainage system slows or gets blocked, pressure rises — and can strain the optic nerve.
Tonometry gives your health care provider a quick and accurate reading of your eye pressure. According to the American Academy of Ophthalmology, normal eye pressure ranges from 10 to 20 mmHg.

Why Eye Pressure Testing Is Important in Diabetes
Diabetes affects the small blood vessels throughout your body, including the ones in your eyes.
According to the Glaucoma Research Foundation, people with type 1 or type 2 diabetes are twice as likely to develop open‑angle glaucoma. This glaucoma risk-diabetes link is one reason regular eye pressure checks are so important. The longer you’ve had diabetes, the higher your risk.
Glaucoma is a group of eye diseases that damage the optic nerve, usually because eye pressure stays too high for too long. The optic nerve carries all the visual information from your eye to your brain. Damage to this nerve can cause blind spots and, if left untreated, permanent vision loss.
Open‑angle glaucoma — the most common form — develops slowly. Closed‑angle glaucoma happens when the drainage angle suddenly becomes blocked, causing a rapid, dangerous spike in eye pressure.
Early glaucoma detection
High eye pressure doesn’t always cause symptoms. Because open‑angle glaucoma develops slowly, you may not notice vision changes until the disease has advanced.
But tonometry helps detect glaucoma early, before vision loss begins. Combined with regular dilated eye exams, it’s one of the best tools for protecting your sight.
However, the tonometry test alone cannot diagnose glaucoma. It’s a helpful screening tool, but a full glaucoma evaluation requires other tests, too, including:
- Dilated eye exam — Look at the retina and optic nerve.
- Gonioscopy — Uses a mirrored lens to see whether the drainage angle is open or closed.
- Pachymetry test — Measures the thickness of your cornea, which affects pressure readings.
- Visual field testing — Checks for blind spots.
So, tonometry is one piece of a complete vision check, not the whole picture.
The Importance of Diabetes Eye Screening Before Vision Loss
According to the Centers for Disease Control and Prevention (CDC), early detection and treatment can prevent more than 90% of diabetes‑related vision loss.
The tonometry test is just one part of early eye disease detection diabetes specialists recommend. It helps reveal pressure changes that may signal glaucoma or other problems, but works best when paired with a complete eye exam. This looks at the retina, optic nerve, drainage angle, and visual field.
Can primary care check eye pressure?
Many primary care doctors offer eye pressure screening as part of routine diabetes and eye health care. This makes it easier to catch early signs of glaucoma or other eye diseases without waiting for a specialist appointment.
But primary care tonometry is only a screening test, not a replacement for a full eye exam. If your pressure reading comes back elevated, you need to follow up with an ophthalmologist or optometrist. Only these eye care specialists can perform the full set of tests needed to diagnose glaucoma or other eye diseases.
Does tonometry hurt or affect vision?
Your eyes shouldn’t hurt during a tonometry test. It also won’t affect your vision afterward.
Tonometry devices are small, quick, and safe. Some use a gentle puff of air, while others lightly touch the surface of your eye after you have numbing drops. Both methods measure how your eye responds to pressure.
After a tonometry test, you can return to your normal activities right away.
How often should people with diabetes have eye pressure checked?
If you have diabetes, most experts recommend eye pressure checks:
- At least once a year during a comprehensive eye exam.
- More often if you already have glaucoma, high eye pressure, or diabetic eye disease.
People with diabetes should also have a dilated eye exam every year to check for retinopathy and other complications.
If your primary care provider offers eye pressure screening, you may get checked even more frequently, which helps catch changes early.

Tonometry is quick, safe, and essential for protecting your vision when you have diabetes. It helps detect glaucoma early, long before symptoms appear. Now that many primary care doctors offer eye pressure screening, it’s easier than ever to stay ahead of eye disease.
You can protect your eyesight by:
- Asking your doctor about primary care eye screening during routine visits.
- Getting yearly dilated eye exams.
- Keeping your blood sugar, blood pressure, and cholesterol in target range.
- Reporting sudden vision changes right away.
With early detection and consistent care, you can lower your risk of vision loss and keep your eyes healthy for years to come.
