If you did misplace it, however, don’t worry \u2014 there are steps you can take to get a replacement.What to Do If You Lost Your COVID-19 Vaccine CardIf you can’t find your COVID-19 vaccination card, there are steps you can take to replace it.First, you can contact the provider where you received your initial vaccination series. Your provider should be able to access your record. If you received your vaccine from UPMC, you can access your immunization records in several different ways:Accessing the UPMC Patient Portal specific to your region or hospital:MyUPMCUPMC Central PaCole Connect (UPMC Cole)UPMC Western MarylandVisiting UPMC’s Certify Care portal, where you can download a copy of your CDC card.Calling 844-876-2822 between 8 a.m. and 5 p.m., Monday through Friday.If you can’t contact your provider directly, other options include:Contact your state health department’s immunization information system (IIS). Vaccine providers are required to report vaccination information to their state’s IIS.If you live in Pennsylvania, the Pennsylvania Statewide IIS (PA-SIIS) can send you a record of your vaccination. You will receive your vaccination record, not a replacement card \u2014 but a vaccination record can serve as official proof.
